Keep this free card in your wallet, locker or lunch bag. If you are ever hurt on the job, it tells you what to do first and what to write down the same day, while you still remember it clearly.
What you write down in the first hours becomes the start of a clear record, and a clear record helps you get the care and support you need to recover well.

If you are hurt at work
Get care first. Get first aid or see a health provider. Call 911 in an emergency.

Write down the same day
- Date, time and exact place
- What you were doing and what happened
- Who you told, and when
- Witnesses: names and phone numbers
- What hurts, and when it started
Why the first day matters
After a workplace injury, the questions come later: when it happened, who you told, what you were doing. A few lines written the same day answer them clearly, so you can focus on getting better. Every province and territory has its own reporting rules and deadlines, and your province’s page sets them out.
The free Workplace Claim Correspondence Tracker keeps every call, letter and decision in one place, and our workplace injury articles walk you through each step.
